Complete Your Forms
To receive treatment, please complete both the Treatment Consent and Medical History forms prior to your mobile IV service. They take about two minutes and must be submitted before your appointment can be finalized.
If someone else in your group is receiving an IV, please share the link with them as well.
Forms only need to be completed once a year.

Appointment Confirmation
Once your forms are submitted, we will text or call you to finalize your appointment time and complete your Good Faith Exam.
If needed, a medical provider may reach out to clarify your history and confirm the appropriate treatment.
Need Us Faster?
If you need your IV sooner or have not heard from us within an hour, you can text “RUSH” to 214-548-3007 and we will prioritize your request.

On your scheduled day, a licensed RN will arrive at your home, office, or event and administer your IV comfortably and professionally.
Most treatments take about 45 to 60 minutes. You can sit back, get comfortable, and let us take care of the rest.
